Department of Revenue

1. Navigate to the Department of Revenue site
2. Click on the All Actions tab
3. Scroll to the Name & Addresses section
4. Select Manage Names & Addresses
5. Click on the Addresses tab
6. Add or edit mailing address to: 9450 SW Gemini Dr PMB 73938 Beaverton, OR 97008
